HOW DO I SCHEDULE AN ASSESSMENT? WHAT CAN I EXPECT DURING MY APPOINTMENT?
To schedule a Rule 25 Assessment, please call 218-749-2877 or send us an email from our Contact Us page and request a phone call from our scheduling department.
Please be prepared with the following:

Social Security Number

Demographic Information (address, marital status, etc)

A valid contact number

Referral Source (Probation Officer, Social Services, etc.)

Employer
-
Emergency Contact
-
Insurance/Funding Information

Date and Location of previous assessments

During your assessment you can expect to:

Answer a number of questions, including but not limited to chemical use history, family history, medical history, mental health history, criminal justice history, etc

Provide your assessor with contact information for your Probation Officer, Social Worker, and any additional referral source

Provide your assessor with at least two collateral contacts

Submit to a urinalysis at the end of the appointment
A Rule 25 Assessment appointment will last approximately 2 hours. Please arrive 15 minutes early in order to complete all necessary paperwork prior to your appointment. Be prepared to give a urine sample by the end of your appointment.
