HOW DO I SCHEDULE AN ASSESSMENT?  WHAT CAN I EXPECT DURING MY APPOINTMENT?

To schedule a Rule 25 Assessment, please call 218-749-2877 or send us an email from our Contact Us page and request a phone call from our scheduling department.

 

Please be prepared with the following:

  • Social Security Number

  • Demographic Information (address, marital status, etc)

  • A valid contact number

  • Referral Source (Probation Officer, Social Services, etc.)

  • Employer

  • Emergency Contact

  • Insurance/Funding Information

  • Date and Location of previous assessments

During your assessment you can expect to:

  • Answer a number of questions, including but not limited to chemical use history, family history, medical history, mental health history, criminal justice history, etc

  • Provide your assessor with contact information for your Probation Officer, Social Worker, and any additional referral source

  • Provide your assessor with at least two collateral contacts

  • Submit to a urinalysis at the end of the appointment

A Rule 25 Assessment appointment will last approximately 2 hours.  Please arrive 15 minutes early in order to complete all necessary paperwork prior to your appointment.  Be prepared to give a urine sample by the end of your appointment.
